Completing forms in the app

In Salve, there are two types of forms:

  1. Enforced forms, which lock your home screen until they are successfully submitted
  2. Other forms, which you can find by tapping on ㆔ > Forms

As long as a form appears in your account, in either of the above ways, they are a requirement from your clinic and must be filled in and submitted.

Enforced forms

When you first log in to the Salve app, you may be prompted on your home screen to enter some details.

Tap on the Enter your details button on your home screen to start, which will take you to your clinic's form.

Once you've answered all the required questions, you can review and edit your answers if needed before submission.

Please check your answers thoroughly, as once this form is submitted it will be sent on to your clinic – and in some cases, update the information held in your patient record in your clinic's system.

Once you are happy that all the information is correct, tap the Submit to clinic button at the bottom, then confirm in the pop-up that appears.

Other forms

Other forms that may have been sent to you by your clinic can be found by going to ㆔ > Forms. Here, you will see a list of forms your clinic would like you to fill in.

The process of filling in the forms is the same as above.

Viewing submitted forms

Once a form is submitted, you cannot amend and resubmit it – but you can still view it and the answers you gave by going to ㆔ > Forms, then tapping on the form you wish to view.

Amending your answers in a submitted form

If you want to amend any information at a later date, please get in touch with your clinic directly.
