Completing forms in the patient web portal
You can view and complete any forms assigned to you by your clinic via the Salve patient web portal, in addition to the Salve mobile app.
- Go to https://patientwebportal.salvehealth.com and enter your email and password to log in
- Once logged in, click on the Forms menu option in the navigation bar
Starting a new form
Click on the Start button next to the form.
The form will then open with the questions your clinic would like you to answer.
ℹ️ If you can't complete a form in one sitting, use the Save & finish later button at the bottom to save your progress and continue another time.
After answering all the required questions, you'll reach the final screen where you can review your answers and finally submit the form.
⚠️ Check the information you've entered is correct, as once submitted this will be sent to your clinic and in some cases update your patient record in their system directly. You will not be able to edit and resubmit answers post-submission.
Once you are happy that all the information is correct, tap the Submit button at the bottom.
A confirmation will display once your form has been submitted successfully to your clinic.
Resuming a form
To resume filling in a form you have not yet submitted to your clinic, click on the Continue button next to the form to be taken through to where you left off in the form.
Viewing a submitted form
To view a form that you have already submitted to your clinic, click the View button to open the form. You can expand each section to see the answers you gave.
Amending your answers in a submitted form
If you want to amend any information in a submitted form, please get in touch with your clinic directly.
